EMAS HCP Local Information

 

Dr Ben Milton, DDLMC Medical Director and Chief Executive, recently met with Dr Leon Roberts, Executive Medical Director for EMAS to raise concerns about ambulance response times for General Practice and patients being in a ‘place of safety’. Dr Leon Roberts, has assured Ben that ‘place of safety’ does not exist.

Dr Roberts further states: The clinician with the patient is best placed to call the HCP line to book any emergency conveyance for any patient and additional clinical information can be added to be highlighted to the control room clinicians. The call taker is not a clinician and so will not interpret things like BP and SpO2 and GCS, etc.

Rising Cost of Energy

 

2022 has seen the fastest rise in the cost of energy for decades and the financial pressure for business is huge, including General Practice.

The increase in the global sum for Primary Medical Services contracts (GMS/PMS/APMS) has not been increased to reflect rising business costs. General Practice is currently tied into a 5 year contract and unlike most other privately owned business cannot increase pricing to cover additional cost.

DDLMC continue to support both GPC and BMA in raising this urgently with Government. We are assured by GPC that this has been escalated and is awaiting Government response. We have spoken to the Joined Up Care Derbyshire ICB locally and they are also pushing the issue at regional and national levels.

We will continue to champion this risk to practice in all appropriate forums. If you would like to share examples of your historic and predicted energy costs with us please do so to ddlmc.office@nhs.net


Access to Deceased Patient Records Policy (supplied by PCIG)

 

Following last week’s PCSE update, we contacted PCIG and they have kindly shared their ​docx icon Access to Deceased patient records policy 2022_520877715.docx This policy is comprehensive and can be edited to add your own practice details. If you have a DPO service other than PCIG, we recommend that your DPO reviews any new data/GDPR policies before implementation.

Practice Responsibilities

As the last registered GP Practice, the practice will be responsible for managing applications for access to deceased patients’ records made under the Access to Health Records Act 1990 (AHRA). See below

As the patient’s last registered GP, the practice will be required to own the AHR requests for deceased patients and must store the record for the required
10 year retention period.

For further guidance on how to keep records, including how long to keep different types of records, please see the NHS Records Management Code of Practice 2021.

Please note - PCSE do not have access to the patient’s digital record and can only provide copies of a Health Record, which they hold in storage.

Task Allocation within Clinical Systems and Improving Communication Flow between System Partners (sent on behalf of Lisa Roberts DDICB)

 

Thank you to all who have be able to create the universal task box 'EXTERNAL TASK TO GP PRACTICE' and/or have shared their bypass number. I can confirm DCHS are aware that your task box has been created (if applicable) in your practice.

Please find attached a revised version of the SOP (V2.6) ​docx icon Task Allocation and Improving Communication Flow _2071762899.docx which now details the latest DCHS information. Please note: A useful suggestion has been put forward by a GP requesting that if the information detailed in Section 4 and Appendix D of the SOP can be published on Pathfinder for those who use this facility. This request has been put forward for discussion at the next Clinical Review Group meeting.

We are under considerable pressure to roll this out across the ICS as soon as possible, so if you have not been in touch please create the Universal Task Box as soon as possible – This will help us start our conversations with DCHS and other future providers.


Primary Care Child and Family Weight Management Extended Brief Intervention Service - Derby City

 

In Derby in 2019/20, almost 1 in 4 children were overweight or obese at the start of primary school and the proportion of children overweight or obese in Year 6 was above the national average (38.6%). Childhood obesity is a local strategic priority and in working towards tackling the prevalence of rising issue in Derby, a new service went live from the 1st April 2022.

The Primary Care Child and Family Weight Management Extended Brief Intervention Service is commissioned by Public Health, Derby City Council and is the first of its kind to be delivered through primary care.

Clinicians from 14 of the participating 19 practices have received a two day motivational interviewing based training with a specific focus on Child Weight Management. In total 20 primary care staff have been trained, comprising a mix of practice nurses, PCN health and wellbeing coaches, health care assistants, and GPs.

Children are routinely measured by the National Child Measurement Programme (NCMP) in Reception Year and in Year 6.

Parent/carers of children that are identified to be ≥ 91st BMI centile by the NCMP, and registered at a participating practice will now be invited for an extended brief intervention within practice to discuss their child’s growth and lifestyle, with a further opportunity for a follow up at 3 months. As part of the intervention, clinicians will refer the child into the local Tier 2 Child Weight Management Programme, Live IT (see booklet attached).

Live IT is a FREE 12 month programme, that accepts referrals of any children ≥85th BMI centile to support with weight management. The programme consists of weekly fitness and healthy eating based sessions, as well as offering support to the whole family. Pay Transparency Update April 2022

Amendments to the GP contract regulations were made that removed the requirement for individuals within scope of the general practice pay transparency provisions to make a self-declaration of their 2020/21 NHS earnings by 30 April 2022.

Individuals within scope of the pay transparency provisions are not required to take any action in relation to their 2020/21 NHS earnings at this stage. Pay transparency remains part of the current regulations, however DHSC has confirmed that commissioners should not enforce the requirement at this time. Currently the individuals in scope of the regulations introduced in October 2021 will need to make a declaration of their 2021/2022 earnings in April 2023 as the provision remains in the GP contract. We continue to request further suspension of the requirement to declare earnings as we believe this is harmful to morale of the profession and could lead colleagues to reduce their working commitments or to retire. We also believe that it is inequitable to single out general practice for this requirement.


Patient Lists

NHSE/I has asked PCSE to recommence data quality checks on GP practice patients lists, this includes a reconciliation of practice patients lists. This work was paused during the COVID-19 pandemic but started again on Monday 1 August 2022. Copies of the communication circulated to practice can be found here: Patient list reconciliation and patient list maintenance.

We have raised our concern with NHSE/I that this is a bureaucratic burden for practice which will detract from their capacity to provide patient care. We asked that the process be delayed until practice had their full complement of clinical and administrative workforce.

Whilst NHSE/I have acknowledged and considered the points raised, they have declined our request, stating that the process will only affect a small proportion of practices nationally and that there will never be an ideal time to restart the process.
