About Us
Swadlincote PCN is a dynamic, forward-thinking network serving a close-knit community across South Derbyshire. We are proud to support innovative services that meet the needs of our local population, including the Team Up (Home Visiting) service, a collaborative model designed to bring proactive, personalised care to patients with complex health and social needs.
We aim to provide a supportive working environment in which clinicians can discuss complex cases, learn from one another and contribute to the ongoing development of the service.
Developing clinical leadership is an important part of our team culture. The successful applicant will have opportunities to build confidence in leadership through multidisciplinary working, supporting colleagues, contributing to governance and quality improvement, and becoming involved in service development.
As part of the role, you will also provide clinical support to the PCN Team working in Ragsdale House. The team is made up of ANPs, Paramedics, Social Prescribers, FCPs, Mental Health Practioners and Health Care Support Workers.
We are based in Woodville, a well-connected village in Swadlincote, within easy reach of Burton-on-Trent, Derby, Lichfield, and Leicester. The area offers a blend of countryside charm, good schools, affordable housing, and a strong community spirit, making it a great place to live and work.

Person specification
Qualifications
Essential
· Full registration with the General Medical Council.
· Current licence to practise.
· Inclusion on the National Medical Performers List.
· Certificate of Completion of Training in General Practice, or an equivalent recognised qualification.
· At least two years’ post-CCT experience in general practice.
· Current UK professional registration.
· Eligibility to work in the United Kingdom.
Experience
Essential
· Experience of working autonomously as a GP.
· Experience of assessing and managing acute, complex and uncertain clinical presentations.
· Experience of making safe clinical decisions where information may initially be limited.
· Experience of working collaboratively with other healthcare professionals.
· Experience of managing clinical risk and recognising when escalation is required.
· An interest in developing clinical leadership skills.
Desirable
· Experience of home visiting or community-based clinical care.
· Experience of urgent care, frailty, care-home medicine or admission avoidance.
· Experience of clinical triage.
· Experience of supporting advanced clinical practitioners, paramedics or other multidisciplinary clinicians.
· Experience of multidisciplinary working across organisational boundaries.
· Experience of leadership, education, supervision, quality improvement or service development.
