You’ll build meaningful continuity relationships with housebound patients, taking a holistic and proactive approach to their care. Working closely with a wide range of health and social care partners, you’ll play a key role in improving outcomes and quality of life for some of our most vulnerable individuals. The role includes a blend of remote work and home visiting, shaped around the needs of the service.

Our Home Visiting Service is a core element of our commitment to the Ageing Well agenda, ensuring frail and housebound patients receive timely, proactive, and personalised care. We employ a dedicated team of Home Visiting Clinicians—including Community GPs, Community Matrons and Paramedics—who deliver high-quality, holistic care for our most complex patients. Embedded within our general practice teams, they play a vital role in improving access, continuity, and patient experience.

We use Team Up, our integrated care model, to support seamless collaboration across practices and services. This approach is central to how we deliver safe, effective, and efficient care in the community.

The Role

We’re seeking Nurse Practitioners, Community Matrons or Paramedics who are:

• Committed to improving access to care for housebound and care home patients

• Skilled in holistic assessment, continuity of care, and clinical decision-making

• Confident working collaboratively across general practice, care homes, community services and the voluntary sector

• Motivated to contribute to service development, innovation and quality improvement

This is a dynamic and evolving role, offering the opportunity to make a real difference while developing professionally within a supportive and forward-thinking team.

Key Responsibilities

• Conduct face-to-face and telephone consultations, including home visits

• Deliver a structured and protected weekly planned care home ward rounds in line with the Enhanced Health in Care Homes (EHCH) model, ensuring proactive, personalised care for residents

• Work autonomously within your professional scope of practice

• Make appropriate referrals and ensure clinical safety and excellence

• Build continuity relationships with patients, carers, and care home staff

• Collaborate with GPs, Community Rapid Response services, Falls teams, and other MDT partners

• Contribute to the ongoing development of the Home Visiting Service, including quality improvement initiatives.

• Participate in daily debriefs, monthly team meetings, and regular supervision

You’ll be supported by Community and practice GPs, with opportunities for upskilling and further training.

What we offer?

• Salary: £46,185.66–£54,711.04 per annum (full‑time equivalent), dependent on skills, qualifications and experience. Salary shown is full‑time equivalent and will be pro‑rated for part‑time hours. North East Derbyshire PCN operates a locally agreed pay framework for PCN‑employed staff. The starting salary will be confirmed at offer stage and reflects the requirements of the role, relevant experience and level of responsibility. Incremental progression may be available in line with the framework, subject to role development, continued funding and review.

• Core Hours: Monday to Friday, 8:00am – 6:30pm

Who We’re Looking For

Essential Criteria

• Registered Nurse or Paramedic (NMC/HCPC)

• Experience in community or primary care settings

• Strong communication and clinical decision-making skills

Desirable Criteria

• Independent prescribing qualification

• Experience working with frail or housebound patients

• Interest in service development and MDT working
