Job summary
To complement our friendly and supportive team, we are looking for an enthusiastic and caring GP to work between 4-6 sessions per week, ideally across Monday, Tuesday and Wednesdays. A competitive salary is offered to reflect experience.
Heartwood Medical Practice is a well-established and highly regarded GP Practice, supporting 11,000 patients from our modern premises in the centre of Swadlincote. CQC rated "Good" at our last review.
We are an innovative practice, continuously seeking improvements in delivering high quality patient care.

Main duties of the job
All GPs have a pro-rata share of routine consultations, duty days and workflow letters and admin tasks.
Patient consultations are a mix of either in-person at the surgery, or remote by telephone or video. Home visits are shared between GPs and the local Home Visiting Service. A centralised PCN team also support the practice and our patients.
We are also a Teaching Practice for medical students from the University of Nottingham and share the mentoring across our GP team.
GPs have protected teaching once per month with locally organised training schedules for some CPD sessions.

We are a friendly and supportive team with a wealth of experience across the team. We have around 40 staff, including 5 GPs, 3 ACP/ANPs, Pharmacist, Practice Nurses, TNA, HCAs, Health coach, Social Prescriber, Care Co-ordinators and reception/admin team.

Job Purpose
The GP will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients according to their individual needs. They will be part of a multi-disciplinary team and will be expected to set an example to others, be involved in liaison with external agencies; assist in education / training and to play a full role in target achievement and service development.
Clinical Responsibilities
The GP will undertake a variety of duties including Practice consultations, both telephone, video and face to face. Handling patient queries, visiting patients at home, checking and signing prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
Admitting or discharging patients to and from the caseload and referring to other care providers, as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards, using the clinical computer system.
Participating in QOF related work.
Compiling and issuing computer generated acute and repeat prescriptions.
Prescribing in accordance with the Practice prescribing formulary.
Perform appropriate skilled examinations with consideration for the patient.
Respect patients right to confidentiality and provide information to colleagues
Respond rapidly to emergencies.
Ensure up to date emergency equipment and drugs used if required.
Ensure up to date with developments in clinical practice and regularly review knowledge and performance.
Use reviews to develop Personal Development plan.
Be aware and comply with practice clinical policies and protocols.

Working within Primary Care Networks: Heartwood Medical Practice is part of Swadlincote PCN and collaborative working is expected.
Awareness of and compliance with all relevant Practice policies / guidelines e.g. prescribing, confidentiality, data protection, health and safety.
A commitment to life-long learning and audit to ensure evidence based best practice.
Contributing to evaluation/audit and clinical standard setting with the practice.
Contributing to the summarising of patients records and read coding patient data
Attending training and events organised by the practice or other agencies, where appropriate.
