Erewash Community GP

We are looking for an additional Community GP for 4 sessions on a Mon/Tues/Wed/or Fri from the beginning of October 2026 due to a vacancy.

The Community GP is a senior clinical post within the TeamUp Community Service. The post holder provides senior clinical leadership, direct patient care and multidisciplinary oversight for patients with complex and frailty-related needs in community settings across Erewash.

The Community GP contributes through three core functions:

•      Senior clinical leadership within the TeamUp MDT

•      Proactive and responsive care for the most complex patients in the community

•      Education, supervision and governance across the service

The working day is structured as follows:

•      08:00 - Start. Attend the daily TeamUp situational awareness meeting. Review current caseload, new referrals, urgent work and service capacity.

•      08:30 onwards - Workload allocation with the team. Allocate patients, identify patients requiring senior input, triage acute referrals.

•      Morning -, ward rounds, triage and MDT clinical support.

•      Midday - Multidisciplinary board round for patients under the service.

•      Afternoon - Follow-up visits, clinical advice to team members, medication review, governance tasks, supervision and training.

•      By 18:30 - Responsive availability for urgent clinical advice and team queries until the end of the operational day.

The GP is expected to be contactable and responsive to the TeamUp team between 08:00 and 18:30. This does not mean 10.5 hours of continuous direct clinical work - it means that the team has reliable senior clinical access throughout the working day.
