Overview
We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our practice team. The ideal candidate will play a crucial role in optimising patient care through effective medication management and collaboration with healthcare professionals. This position requires a strong understanding of pharmacology and patient care principles, particularly in a General Practice setting.
Old Station surgery is a democratic, caring and inclusive practice with teamwork, support and compassion at its core. We take pride in delivering quality care to our patients and have a strong training and personal development ethos.
Serving a patient population of c15,100 in a modern purpose-built premises in the heart of Ilkeston, Derbyshire, the surgery has an on-site pharmacy unit and 2 branch surgeries in the neighbouring areas of Cotmanhay and Kirk Hallam.
Job summary
The Clinical Pharmacist will be responsible for the following:
To play a pivotal role in the delivery of high-quality primary health care
To independently assess, verify and prescribe for patients safely within national and local guidelines
To provide clinical medication reviews for patients with single or multiple long –term conditions where medicines optimisation is required
To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission
To maintain full and complete records of all patient contacts using appropriate clinical templates and coding
To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate.
Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate
Work with practice team to ensure full compliance with Care Quality Commission standards for safe and effective care

Essential criteria:
Registered Pharmacist with the GPhC
Qualified as an Independent Prescriber
Minimum of 2 years post-qualification experience
Desirable:
Experience in primary care setting
Strong clinical knowledge and/or qualification in respiratory conditions or willingness to develop in this area.

NHS Pension
26 days annual leave (plus Bank holidays) – pro rata for P/T employees
Hours: Ideally total 26.5 hours a week to be worked as follows:
Monday 8:00- 18.00 (with 30 minutes unpaid break) - COT
Tuesday 9:00- 18:00 (with 30 minutes unpaid break)- OSS
Friday 9:00- 18:00 (with 30 minutes unpaid break)- OSS
However, we are prepared to offer some flexibility to the above arrangements for the right candidate.
