2018-19 national GMS contract, and Direct Enhanced Services Participation Agreement

NHSE have sent the attached letter and DES Participation Agreement for 2018-19 to all GP practices in Derbyshire and Nottinghamshire They have asked for practices to complete, wet sign and return the Participation Agreement by 31 May 2018 at the latest.
