GP mythbuster 21: Statutory Notifications to CQC - Care Quality Commission

GP practices are legally obliged to notify CQC about certain changes, events and incidents that affect their service or the people who use it.

We consider whether providers have the systems and processes in place to correctly notify us under the safe and well-led key questions. The relevant key lines of enquiry (KLOEs) are:

S6.1 Do staff understand their responsibilities to raise concerns, to record safety incidents, concerns and near misses, and to report them internally and externally, where appropriate?

W6.6 Are there effective arrangements to ensure that data or notifications are submitted to external bodies as required?

S6. Are lessons learned and improvements made when things go wrong?


S6.1 Do staff understand their responsibilities to raise concerns, to record safety incidents, concerns and near misses, and to report them internally and externally, where appropriate?


S6.2 What are the arrangements for reviewing and investigating safety and safeguarding incidents and events when things go wrong? Are all relevant staff, services, partner organisations and people who use services involved in reviews and investigations?


S6.3 How are lessons learned and themes identified, and is action taken as a result of investigations when things go wrong?


S6.4 How well is the learning from lessons shared to make sure that action is taken to improve safety? Do staff participate in and learn from reviews and investigations by other services and organisations?


S6.5 How effective are the arrangements to respond to relevant external safety alerts, recalls, inquiries, investigations or reviews?

 


THE NEUROLOGY ENHANCED TRIAGE VIRTUAL CLINIC

After a successful roll out of the Neurology Enhanced Triage Virtual Clinic across South Yorkshire, the Sheffield Teaching Hospital Neurology department are launching this service for patients referred from DDICB.

All referrals sent to the Neurology service at Chesterfield will be reviewed and triaged by a Consultant Neurologist. For a small number of patients, the Neurology team are able to provide specialist advice directly to the referrer without the need for the patient to attend an appointment. This delivers timelier care for patients with more immediate access to diagnostic tests. Any investigations required will be arranged by the Neurology Department and both referrer and patient will receive written advice with regards to the clinical management.

Referrals should still be sent via the eRS Referral Assessment Service (RAS) for General Neurology at Chesterfield. If the management strategy is not acceptable to the referrer or patient, or if it is felt that the concerns have not been addressed, onward referral to the neurology clinic will still be considered.


CHANGES TO SHARED CARE PATHOLOGY

 
  • The new prolactin guideline has gone on the website today - have a look and do comment and let us know, as always, if there are any changes you think might be helpful to you. It has been developed with endocrinology consultants and the mental health pharmacy team.
  • You might also have noticed that the HbA1c requesting on ICE has been updated. There are two options for requesting HbA1c on ICE which are linked to different reference ranges when reported from the lab (20-48 mmol/mol for monitoring known diabetics or 20-41 mmol/mol for diagnosis of diabetes). Please take care to request the correct test.

DUPLICATE HOSPITAL RESULTS

Dr Penny Blackwell has been working with the pathology team at UHBD regarding duplicate hospital results being copied in to GPs. UHDB are going ahead with disabling the option to 'copy results to GPs' in Lorenzo on 15th Feb.

This should reduce the number of results GPs are receiving from UHDB that have not been requested (especially blood cultures).

 

We are continuing to look into other avenues these instances may occur including results from Chesterfield Royal that may be impacting the GPs in the north.

SERVICE NAME CHANGE IN OPHTHALMOLOGY

Please note that from Wednesday 1 March 2023, the Primary Eye Care Clinic at RDH will be changing its name to Acute Eye Care.

The new email address will be uhdb.acuteeyecare@nhs.net with no other changes to the referral process.

LOCUM GP WORKFORCE PROJECT SURVEY

The aim of this survey is to explore the opinions of locum GPs working in the Midlands on current state of general practice, permanent GP roles and what would encourage them to take up more sessions and/or permanent roles. We want to better understand how we can support locum GPs.

The questionnaire takes about 3 – 5 minutes to complete.

Telephony Baselining Exercise: Survey is still open - Due 17:00 Friday 24 February 2023 – SENT ON BEHALF OF DDICB

We previously sent communications about this survey in November, we are resending this message and asking you to encourage any practices who have not had the opportunity to complete this survey to do soon at their earliest convenience.

Work is ongoing to establish a national procurement framework for advanced (cloud-based) telephony solutions, planned to be live from January 2023. This will support you in the future procurement of a telephony solution, ensuring value for money investment in a fit for purpose solution that meets the national specification. There may also be an opportunity to access additional revenue funding (to be confirmed) to support you with buy-out costs should you wish to exit your existing contract early.

Whilst some information is known on the solutions in use and associated exit costs this does not cover every practice in England. It is critical that the national team have a complete picture of the national telephony estate, and in particular exit costs, to assess the requirement and support access to potential additional funding. The data gathering exercise does therefore include a question related to contract exit costs which we urge you to complete if possible.

Please compete this short baselining questionnaire, although there is no strict deadline for this return, it would be appreciated if you could complete the return by 17:00 Friday 24 February 2023. The questionnaire has been pre-populated with any information you may have provided in the past which should be checked and amended, as necessary.

We would like to make practices aware that bookings are now open for the annual Darling Bugs of May Conference – Infection & Prevention Control. This will be held at Trent Vineyard on 17 May. This conference includes numerous speakers from the Infection Prevention and Control Industry.

A wide range of subjects will be covered including IPC and human factors, IPC in mental health settings, community and acute focus.
