Job summary
We have an exciting opportunity for a General Practice Nurse to join our friendly supportive, high performing team at Hannage Brook Medical Centre. The role is advertised to cover a planned absence within our current team. We are hoping to recruit a candidate as soon as possible for a minimum duration of 6 months ( but possibly longer). The role is flexible between 15 to 20 hours per week.
Please note we will review and interview suitable candidates as applications are received which may mean we close the advert earlier than advertised.
There are options to participate in our extended access clinics in the evening and Saturdays if additional hours are required.
Main duties of the job
The successful candidate will need to demonstrate the ability to effectively manage respiratory services within the surgery and provide nursing services to patients including childhood immunisations, vaccinations and health screening.

There is some flexibility in when the hours are worked.

Job description
Job responsibilities
To provide general practice nursing services to patients of the practice including:
To assess the needs of patients and provide appropriate care or treatment, in conjunction with a GP or independently, as appropriate, and according to practice policies and protocols
To assist in the provision of care to patients with chronic diseases and to undergo any training required to become proficient in these areas as required by the practice.
To undertake a range of general nursing duties, to include wound care, ear syringing, venepuncture, electro-cardiograph, giving immunisations and vaccinations, and any other procedures such as have been agreed with the practice and to undertake the training required to perform these tasks
To undertake health screening procedures, eg. cervical screening, and give patients appropriate health promotion advice
To document the process of assessment of patient need and delivery of care
To evaluate the outcome of care and where appropriate identify changes that may need to be made to an individual patients care management
To assist in the provision of care for housebound patients with chronic diseases to the same standards as those able to attend the surgery.
To take active role in asthma and COPD care and manage patients with support from local Impact Plus respiratory team

RN currently on NMC register
Evidence of continuing professional development
Driving licence and car owner
Diploma or extensive experience in Asthma and COPD management
