Job Description

North East Derbyshire PCN are recruiting for Community GP to support our multi-disciplinary team providing healthcare to our housebound patients and patients in care or residential facilities across the PCN footprint.

This job may particularly appeal to those with school age children or commitments at the start/end of the day – we are looking for someone to take on the role to work 5 hours in the middle of the day eg 10-3, 5 days per week, although we are willing for this to be flexible depending upon your commitments. Job share and part time applications are also welcomed.

We envisage that the role will involve developing continuity relationships with our housebound patients, taking a holistic approach to their management, and liaising with other health and social care organisations to improve the care of these vulnerable individuals. This will be a mix of remote and visiting work depending on the needs of the service.

There will also be a leadership aspect to the role, providing support and supervision to the rest of the Home Visiting Team and helping to develop and evolve the service over time.

You will be supported in the role by GPs at the four practices within the PCN and our fantastic existing Home Visiting Team currently comprising of 2 Community Matrons, 2 Paramedic ACPs and a dedicated Care Coordinator.

 

About us

North East Derbyshire PCN is an organisational group of four likeminded GP practices in North East Derbyshire. The Practices are Barlborough Medical Practice, Killamarsh Medical Practice, The Springs Medical Partnership and The Valleys Medical Partnership.

We are a relatively small PCN with just over 40,000 patients. We feel that this makes us big enough to develop a successful team but small enough to be an approachable, friendly place to be.

The NED PCN team is committed to working collaboratively to improve the care of patients in North East Derbyshire and sustaining General Practice.

All our practices are close to the M1 and commutable from major cities including Chesterfield, Sheffield, Derby and Nottingham.

 

 

Key Duties Tasks and Responsibilities

  • Provide day-to-day clinical leadership for the multi-disciplinary team including debriefing after visits if needed
  • Home visiting for clinically complex cases e.g., end of life care, diagnostic uncertainty, complex prescribing decisions
  • Senior clinical triage for the Acute Home Visiting service
  • Build relationships across the extended team including, but not limited to social care, ambulance, social prescribers, community, and mental health clinicians.
  • Opportunity for involvement in service development as the team expands
  • Involvement in MDT meetings
  • Provide holistic comprehensive assessment and enhanced advance planning
  • Maximise best care in the patient's own homes to reduce the need for hospital or care home admissions.
  • Provide end of life care in line with the patient's wishes
  • Quality improvement activity
  • Opportunities for teaching and mentorship

 

 

Person Specification

Aptitude and Personal Qualities

Essential

  • Flexible, supportive, collaborative.
  • Recognise the benefits of multiagency & multidisciplinary team working.
  • Ability to work as part of a multi-disciplinary team and lead a team

Desirable

  • A passion for excellent, holistic, patient centred care for older people with frailty

Qualifications

Essential

  • A vocationally trained and accredited GP
  • Current registration with GMC
  • On the GP performers list

Desirable

  • MRCGP
  • Recognised qualification in Care of the Elderly
  • Recognised qualification in medical education/clinical supervision
  • Evidence of leadership development

Experience

Essential

  • Experience and evidence of an interest in care of the elderly
  • Understanding of adult safeguarding and deprivation of liberty procedures
  • Experience of multidisciplinary working
  • Experience of senior clinical triage

Desirable

  • Knowledge and experience of carrying out Comprehensive Geriatric Assessment
  • Experience of medical education/clinical supervision
  • Experience of using SystmOne
  • Experience of working in a multi-disciplinary team.

Salary

£10,500+ per session depending upon experience
