Job Summary

To provide high quality nursing services including Chronic Disease Management (such as: Asthma, COPD, Diabetes), Learning Disabilities reviews, Long Term Condition reviews, NHS Health Checks, ECGs, blood pressures, bloods, foot checks, urine analysis, spirometry, ear syringing, cervical cytology, wound care and minor injuries management, general health and lifestyle advice, vaccinations, injections and immunisations.

 

To be actively involved in health promotion to the Practice population in collaboration with the Primary Health Care Team, meeting the needs of the patients and supporting the delivery of the Practice Policies & Procedures.  The post holder will work within their professional competence in accordance with the NMC Code of Professional Conduct and Scope of Professional Practice.

 

To work under indirect supervision and perform tasks prescribed and delegated by the GP Partners, Practice Manager and Nurses working to PSDs, PGDs, agreed protocols, within an evidence base and within defined competencies to meet the needs of patients.

 

To liaise with the Practice Pharmacy Team, Community Staff, Practice Nursing Team plus other healthcare professionals and to working closely with the Management, Administration and Care Navigation teams.

 

Primary Responsibilities

Professional Responsibilities

  • To work within NMC Guidelines and to agreed practice protocols.
  • To maintain high standards of clinical practice by ensuring the application of available evidence based research and agreed protocols.
  • To work in collaboration with the Primary Health Care team to address identified individual and population needs.
  • To identify and address own learning needs by participation in the processes of personal professional and practice development planning.
  • To access clinical supervision and provide mentorship to trainee Nursing team members.
  • To maintain accurate and contemporaneous patient records and registers in accordance with practice policy.
  • To maintain confidentiality in accordance with NMC and agreed Practice and Information Governance policy.

 

Clinical Responsibilities to include

  • To manage treatment room consultations such as minor injuries, wounds, dressing and treatment of leg ulcers, suture removal, ear examination and syringing.
  • Administer routine injections in accordance with the practice policy and appropriate protocols, PGDs and PSDs ie. Childhood and adult immunisations in accordance with national guidelines such as: Flu vaccinations, travel vaccines and completion of patient vaccination records. Anti-psychotic injections, Vit B12 injections and contraceptive injections.
  • Contribute to Chronic Disease Management in areas such as: Asthma, COPD, Diabetes, CVD Checks, Learning Disabilities, Long Term Conditions and NHS Health Checks and to other Enhanced Service specifications as required.
  • Provide women's health services including cervical cytology and advice on contraception and the menopause according to practice policy.
  • To undertake investigative procedures including ECG, blood pressure, urine analysis, foot checks.
  • To prepare and assist with minor surgical procedures.
  • To prepare and assist with coil and implant procedures.
  • Accurately label and document patient samples and submit for collection and analysis.
  • Contribute to Nursing Team record keeping: clinical, administrative, H&S related records.
  • To assist in and perform routine tasks related to patient care as directed by GPs and the Management Team.
  • To be responsible for the assessment of health needs of patients and the provision of care/treatment in conjunction with the GP, other members of the PHCT or independently, undertaking ongoing evaluation of the care/treatment outcomes, modifying practice as necessary and documenting progress.
  • With support implement and evaluate individual treatment plans for Chronic Disease patients that promote health and wellbeing.
  • Chaperoning duties.
  • Partake in audit as requested by the Audit Lead.
  • You will be expected to participate in the induction plan for all new staff members whatever their role may be as requested by Management.
  • You will be expected to take part in the training programme for any new or role changing staff members as requested by Management.

Secondary Responsibilities

Additional responsibilities

  • To liaise and refer to other health care professionals, attend and participate in practice meetings on a regular basis, report any areas of concern, prepare purchase orders in conjunction with the admin team for ordering of drugs and clinical supplies.
  • Rotate refrigerated and non-refrigerated stock according to date, maintain appropriate records and discard out of date stock in accordance with practice protocol.
  • Respond to cold chain incidents and report effectively and efficiently using national and local guidance, including significant event reporting.
  • Contribute to care and maintenance of equipment including cleaning and disinfecting items of equipment and keeping an up-to-date record.
  • Support the repair and regular maintenance and calibration of equipment, including equipment maintenance / checking / calibration log.
  • To support and participate in the development and implementation of practice policies and protocols.
  • To identify appropriate leaflets for health promotion, ensuring adequate stocks are available, keeping leaflet holders in the waiting room stocked in conjunction with the Health Care Assistants.
  • To attend and participate in practice meetings/training and external meetings/training on a regular basis.
  • To develop effective communication pathways with all members of the practice team.
  • To provide a friendly, happy and positive working environment for both staff and patients.
  • To work with other members of the team towards the achievement of targets defined in ‘raising standards in primary care’.
  • To ensure your treatment room is fully stocked and clean at all times.
  • To ensure infection control guidelines are reviewed and maintained.
  • To assist in and perform routine admin tasks related to patient care as directed by GPs and the Management Team.
  • To undertake regular mandatory training and undertake continuing professional development to ensure competency for delivering all responsibilities.

Induction Training

On arrival at the practice all personnel are to complete a practice induction programme; this is managed by the Operations Manager.

 

Learning and Development

The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills, knowledge, attitude and competences to perform their role. All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the practice training programme.  Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately, enable them to improve processes and service delivery.

All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner.

 

Service Delivery

Staff at Newhall Surgery must adhere to the information contained with practice policies and regional directives, ensuring protocols are adhered to at all times. Staff will be given detailed information during the induction process regarding policy and procedure.   

 

Security

The security of the practice is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.

 

Professional Conduct

At Newhall Surgery, staff are required to dress appropriately for their role. Administrative staff will be provided with a uniform whilst clinical staff must dress in accordance with their role.
