Practice Nurse Vacancy

This is an exciting time to be joining Moir Medical Centre (part of Erewash Health Partnership). In addition to the services in Erewash that support our Practices such as an Acute Visiting Service, a Nursing Home Service and extended evening and weekend clinics, we are expanding our Practice team to further enhance our access, quality and experience for patients.

With our values set in being patient focussed, flexible, customer orientated, and a team player with a ‘can do’ attitude we are looking to recruit a highly motivated and enthusiastic Practice Nurse, who can demonstrate a high standard of clinical excellence.

We would welcome applications from nurses who are seeking a role, part time or full time (25 to 37.5 hours flexible).

The ideal candidate would have experience of working in primary care, all aspects of long-term conditions, chronic diseases and provide provision for preventative healthcare. These criteria are not essential as we will provide training and support.

Our experienced and valued nursing team consisting of Phlebotomists, Health Care Assistants, Trainee Nurse Associates and Practice Nurses, all providing support and care to our patient population.

Moir Medical Centre provide care from our three sites (Long Eaton, Sawley and Toton) serving a population of circa 13,000 patients, and we are fully part of the leading-edge Erewash Health Partnership (Primary Care Provider) that provide care collectively to over 75,000 patients, and the PCN at over 103,000.

As a training practice on Nottingham and Derby GPST, we currently have GP registrars training with us.

You will work effectively as a team member, supporting and communicating with other professional colleagues and clinicians, ensuring clinical practice are safe, effective and competent.

Main Job Responsibilities:

  • Chronic Disease management (Diabetes, Asthma, COPD, Hypertension, CVA, CHD)
  • Cytology
  • Assist in the achievement of QoF
  • Assist in and perform routine tasks related to patient care as directed by GPs / Nurse Manager

Current NMC registration

Relevant post registration experience of working in a primary care environment i.e. practice or community nursing

Experience and or evidence of relevant study in chronic disease management

Broad knowledge base & relevant clinical knowledge

 

ü

Excellent communication, listening & interpersonal skills

ü

 

IT skills (SystmOne, Internet etc)

 

ü

Effective time management

ü

 

Working as a team

ü
