Patient Online and forward planning with end of life care and long-term conditions

This workshop will provide an opportunity to learn more about how Patient Online can be used as part of a person-centred approach to care that empowers patient and carers.

Learning outcomes

1. What safe and effective record access looks like and why it's important and useful.

2. The ways in which record access enables a person-centred approach to care and how it can be used in forward planning with patients.

3. Sharing of practical examples and experiences from colleagues working in general practice and how Patient Online can be used as a clinical tool
