Derbyshire Health and Care System Critical Incident Declaration
The Derbyshire system declared a Critical incident on Wednesday evening and continues to see very significant pressure within the system. Please see the staff update FINAL JUCD Critical Incident Staff Brief 210722.pdf
This has come about due to the impact of the hot weather against the background of significant pressures that were already in the system. In the absence of available social care provision some patients are having to be discharged from our acute hospitals without the desired care in place at the time of discharge so please be aware of this. Families and carers are being encouraged (and in some cases recompensed) to support discharges in these situations to try and maintain flow and allow for the ongoing admissions.
We know that hot weather increases health care demand and one of the groups most at risk of admission are those with chronic respiratory conditions. As highlighted in the update patients and their carers have been encouraged to ensure they have the correct medication and knowledge of how to use it. Please could you try wherever possible to proactively communicate with this cohort to ensure they are given the best possible chance of avoiding an admission. ARRS staff (in particular Pharmacists, Pharmacy Technicians and Care Coordinators) are particularly well placed to be able to do this so please enable them to focus on this work in the short term wherever possible.
We fully appreciate the pressure that still exists within General Practice as well and are grateful to everyone for their efforts and the fantastic service you continue to provide despite all the recent challenges.
DRRB Pay Rise BMA Update
We are aware of the latest press announcements of a 4.5% pay rise for NHS staff, we were only aware of this as the news broke.
Many of your employees will have heard the news and will be asking questions about the pay rise. As GMS/PMS providers you are independent contractors & businesses and the decision to award a pay rise will be based on your individual practice finances. There are many considerations to make when considering pay rises. How it will affect pension, NI and tax contributions, aligned with staff wellbeing and retention.
As independent contractors you are not bound by the Terms and conditions of Agenda for change, we do strongly urge you to check your Employment contract terms and conditions surrounding pay rises for your employees.
Over one million staff under the Agenda for Change contract, including nurses, paramedics and midwives, will benefit from a pay rise of at least £1,400 this year backdated to April 2022. This is on top of the 3% pay rise they received last year, despite a wider public sector pay pause.

CQC Remote Searches Update July 2022
The CQC have released an update in preparation for potential future CQC inspections. There is an updated CQC YouTube presentation on the remote searches carried out by CQC during a practice inspection. https://www.youtube.com/watch?v=yGshX-w98yU

COVID-19 Autumn Booster and Flu Vaccine Programme Expansion

We have raised our concerns and questions at DDLMC with the BMA/GPC. We have attached here DHSC Announcement 2022-23 flu seasonandCovid booster0718.pdf a link to a letter that Surrey & Sussex LMCs have sent to their practices. We at DDLMC agree contents with the letter and are sharing with our practices.
The Seasonal Flu Immunisation Enhanced Service has not yet been published, although is awaited, but is unlikely to include any major changes in terms of logistics, delivery and vaccine administration. We will circulate this Enhanced Service as soon as it is available and the LMC.
The current LMC understanding is: There is no central stock ordering supply planned for this year and therefore practices will need to order via the suppliers listed in this document: B1868_Reimbursable-vaccines-and-eligible-cohorts-for-the-2022-23-NHS-Seasonal-Influenza-flu-Vaccination-Progra.pdf (england.nhs.uk)
The additional groups set to be offered the free flu vaccine in England will be:
- all adults aged 50 to 64 years
- secondary school children in years 7, 8 and 9, who will be offered the vaccine in order of school year (starting with the youngest first)
The NHS will announce in due course when and how eligible groups will be able to book an appointment for their COVID-19 autumn booster, and when people aged 50 to 64 years old who are not in a clinical risk group will be able to get their free flu jab.
People in these groups are asked not to come forward until further information is announced.

Expression of Interest - GPTF Training and Development Non-Clinical, Reception, Admin & Practice Management Trainers
We are inviting Expressions of Interest to join a small team of individuals that can design, develop and deliver training for our General Practice workforce. This will include non-clinical roles such as admin, reception and management.
GPTF already has an excellent track record delivering our Practice Management Induction and Step-Up Practice Management programmes. We hope this provides an exciting opportunity to expand to other admin and non-clinical workforce roles which adds to our portfolio of clinical education and training.
We’d love to hear from you if this is something you are interested in, whether you have limited capacity or infinite hours, please complete the attached EOI and return back to me by Thursday 21st July.
Polite reminder: Patients booking a UHDB blood tests
Since last year, the option has been available for UHDB patients to book themselves in for a blood test (opens in new window) > online, via the Switftqueue booking system. UHDB patients will be able to book an appointment at our blood clinics listed on our Swiftqueue homepage. However, please note that the service provided at both Samuel Johnson and Sir Robert Peel Community Hospitals is only for patients who have had their bloods ordered by secondary care - patients referred by their GP will not be seen.
Patients should be using the Swiftqueue system in the first instance and should only contact the Patient Access Team (adults appointments only) or Paediatric Reception Team (children appointments only) if there is a booking system error. These teams only have capacity to manage exceptional circumstances and should not be treated as the default service.

NATIONAL UPDATES
Vaccine certificates to be available from the 21st July for those under 16 who don't have the app or NHS logins
We've heard from NHS X (Transformation directorate) that a new service is due to start on 21st July that enables children to be issued with electronic copies of their covid vaccinations via the web. Up until now those 13 and older could have used the NHS login process and/or the NHS app to get them and those 12 and older could have requested paper copies to have been posted out to them.
The new system will rely on there being a mobile number and/or an e-mail address associated with the child's PDS record.
To get a pass, the name/DOB/mobile must be matched and a short code will be sent (proving the phone is in the possession of the user) and then an e-mail address can be entered to which the pass will be sent.
For anyone with an e-mail contact detail only but no mobile, the pass will be e-mailed to that address once the name/DOB/e-mail address match has occurred.
People can still ring 119 (or go online) to ask for paper copies to be posted out.

The only potential work for GPs would be in updating mobile numbers on patients' records if the numbers were out of date.
Phishing Attempts
This week, we have witnessed a significant increase of phishing attempts to healthcare organisations. The 'themes' relate to Office 365 (SharePoint) and NHSmail. See examples below:
The notifications are sent from several 'compromised' NHSmail accounts to appear legitimate.
Do not reply or click on any links.

- As referenced in the guide, forward - as an attachment - any phishing emails to spamreports@nhs.net.
- Easiest way is to highlight the email and press CTRL+ALT+F on your keyboard.
Our intelligence services also indicate that threat actors are currently targeting finance departments/financially responsible leads.
Please do not be afraid to report any incidents or malicious activity that you have either clicked on or shared credentials with.
GPAD PCN Dashboard
We wanted to make the PCNs in your region aware that a new GP Appointment Data dashboard which is available to them.
