NHS Improvement consultation on 'Shaping the future of patient safety investigations'

The Serious Incident framework (2015) describes how Serious Incidents should be reported and investigated in the NHS. 
NHS Improvement are seeking views on how their guidance could be revised to support the system to respond appropriately when things go wrong.

The closing date for the consultation is 12 June 2018.
