Introducing a GP Workshop - Co-Formation

A GP’s guide to Social Prescribing, Care Navigation, Signposting, PAM’s and Care Planning

Over the last few years several new terms and roles have entered the primary care vocabulary; Social Prescribing, Care Navigation, Signposting, Patient Activation Measures and Care Planning. So, what do they all actually mean and what is their relevance to everyday general practice. In the GP 5 year forward view they underpin several of the 10 high impact actions devised by NHS England to “free up more time to care.”

Co-Formation is owned and run by Dr Joanne Cummings MBBS MRCGP and Mrs Sarah Pearce. Joanne is an experienced GP of over 15 years and recently the Clinical Lead for the Complex Care Service at South Somerset’s Symphony Vanguard Project. Her role here involved implementing New Models of Care and new roles to support General Practice. She has also worked as a Clinical Lead for Collaborative Care and Support Planning at the RCGP. Sarah is a former practice manager and previously Health Coach Coordinator in the Symphony Project, she has expertise in social prescribing, care navigation and their implementation.

This GP Workshop has been developed to support GP’s to incorporate Care Navigators, PAMs and Care Planning into their practice. There are a number of courses available for training practice staff in signposting and care navigation, but their successful implementation will depend on how their roles are utilised by clinicians. These new roles and process are designed to increase sustainability and reduce gp workload, as well as promoting person centred care.

They aim to explain using illustrations from around the country to enthuse GP’s about how their practices could be transformed. They will also cover areas of concerns such as clinical oversight, indemnity and also explore any local developments or plans for new models of care. They recommend a 3-hour session which will be interactive with an accompanying electronic handout and useful links and further information will be included.
