Guidance on Death Verification Certification and Completion of Cremation Form 4
I thought GPs would be keen to see the finalised version of the updated LMC Guidance. The processes concerning verification and certification of death and the completion of Cremation Form 4 has been the subject of much debate, both locally and nationally. We are now finally in a position to publish what will hope will be our definitive guidance on these matters. We are awaiting a GPC/BMA update and will publish any amendments although we are confident that the two sets will align.

A copy of the letter sent to the London Coroners by Dr Mark Sanford-Wood Deputy Chair GPC in response to their letter which shows that the GPC/BMA are fully aligned with the view of DDLMC.
