Primary Care Feedback Event – Referral Process

Times: 12:30 - 13:30

We are about to embark on a three month review following the implementation of the above service which commenced in December 2020. The service was commissioned and aligned to NHSE/I Guidance. It supports people who are encountering ongoing symptoms having had Covid after 4 weeks and onwards (known as Post Covid Syndrome).

If you have used the service or not, we are really keen to understand more. We greatly value hearing your views and experience, including any difficulties you feel accessing the service. This is really important to assisting in the continuous improvement of the service and patient experience. We are working alongside colleagues at DCHS, (the service providers,) to ensure we understand how the referral process from primary care is working. We would welcome your comments on any issues you have encountered regarding the process, including:

  • use of the Practice Information Pack (attached)
  • the referral form
  • the diagnostics required
  • patient questionnaire
  • use of the recommended Snowmed codes

This session will provide an opportunity for you to speak or input your views into the comments box.
