Debt and Mental Health Evidence Form
Patients who have a mental health condition may ask you to complete a Debt and Mental Health Evidence Form (DMHEF), which will act as evidence and support them in seeking help with their repayment of finances. You can find guidance from the BMA here. This can be completed by a wide range of health professionals and shouldn’t default to the GP. Advice from BMA GPC advises you are only obliged to answer the following:
- Does the person have a mental health problem? Yes/No
- What is this mental health problem? If it has a name or diagnosis, what is it?
In addition there is a strong emphasis with debt agencies that they should accept self declaration statements from patients.
