COVID-19 SPECIAL 21.05.2020

JOINT DDLMC/ GPA / GPTF / DCHS COVID RESPONSE TEAM

CPR AND PPE. Dr Duncan Gooch has been leading on this issue and has asked us to share the following with you. We would urge you to respond as requested please:

Dear Practice,

As I am sure that you are aware the Resus Council UK and PHE disagree whether chest compressions are an aerosol generating procedure.

We feel that it is prudent to follow the Resus Council’s guidance for CPR in primary care settings. https://www.resus.org.uk/media/statements/resuscitation-council-uk-statements-on-covid-19-coronavirus-cpr-and-resuscitation/primary-healthcare/

In this, it is recommended that level 3 PPE is the safest option for AGPs, but it acknowledges that there are circumstances where it may not be possible to use it. In all circumstances, individual healthcare worker’s decisions must be taken based on the specific circumstances that they find themselves in.

It seems reasonable that individual practices consider stocking their resus trollies with level 3 PPE. We would like to support that process.

Level 3 PPE includes a FFP3 mask which would require training and FIT testing. Practices will need to consider how they might provide cover throughout the week to use this equipment. Note that certain people might not be able to use FFP3 respirators eg those with facial hair.

In order to support practices, please could you respond to ddccg.ppe@nhs.net with the answers to these questions

  • Practice name
  • Number of sites that require equipment
  • Number of healthcare workers that will require training
  • Number of existing trained staff

Many thanks for your responses

Duncan Gooch

MANAGING CAPACITY AND DEMAND – A GREAT OPPORTUNITY

As part of the pandemic response there has been a central data extraction of COVID19 coded activity in practices which has been extremely powerful in showing the valuable contribution that general practice makes and provides a strong voice in the planning of work. The GP Alliance are keen to develop a system across Derbyshire that would enable the production of such data, routinely for other work in general practice. Ultimately this would give general practice the ability to trigger OPEL levels in line with most other providers in the system and add weight to the GP position.

order to develop the system we are seeking some practices (SystmOne only at the moment) with an interest in this area to develop a pilot scheme that could then hopefully be rolled out to all practices. The full details are at Attachment A and any interested practices should email ddlmc.gptf@nhs.net by 1st June please.

AND FINALLY…

Following my moment of marathon madness yesterday, I’ve been overwhelmed by the donations and very kind messages. Thank you all very much.

pleased to report that as things stand, I’m looking forward to a nice restful day on Saturday as we haven’t reached the target yet – PHEW………Unless some more people want to donate ……..
